There are two parts to Wellness for Life. One saves you money in the form of a percentage discount off the cost of your insurance. The other part helps you live a healthy lifestyle with services from Mayo Clinic Health Solutions.

You’re qualified

Wellness for Life is a rider available on most new life insurance policies, regardless of your current health or weight. It’s available for a onetime fee of $100. Depending on your policy, Wellness for Life may pay for itself after a few years.

Your Rewards

There's two levels of rewards. You can save a percentage of your insurance costs every year, based on two things.

1. Level one rewards are received by simply having a biannual physical by your doctor. Have the doctor state your weight on a form and return it to us.

2. You will receive twice the Rewards when you have your physical and maintain the weight (within a range) you had when your life insurance policy started.

Your health

Once you’re enrolled in Wellness for Life, you can use Mayo Clinic EmbodyHealth Web site as a resource for assessing, tracking and improving your health. It can help you stay within your weight range and maintain your rewards.

Questions and answers

Q: Will my premiums go up or down as my health changes?
A: No. Aviva will not be re-evaluating your health risk or re-underwriting your policy. Nor is it lawful to do so. These benefits are meant to be used only by you for assessing and tracking your health. And Aviva will not have access to any personal information gathered as a result of the Mayo Clinic Health Assessment. The only information we ask for is your doctor certified weight (at least every two years) to determine your qualification for level two rewards.

Q: Is Wellness for Life available to current life insurance policyholders?
A: At this time we are able to include Wellness for Life only on select new life policies. It’s unlawful to re-assess or re-underwrite a policyholder, so we can not add it to current policies.

Q: When will my benefits be available?
A: The Mayo Clinic EmbodyHealth Web site is available as soon as you receive your log-in information, a few weeks after your policy takes effect. If you meet the criteria outlined in “Your Rewards,” in the second year of your policy, your cost reductions will take effect in the third year.

Q: I’m over weight. Is Wellness for Life for me?
A: Yes. In fact, if you anticipate your premiums might be higher because of your weight, your rewards could be greater, because your savings are a percentage of your life insurance costs.

Q: What happens if I stop going to the doctor and reporting my weight?
A: You’ll lose the Rewards for that time period, but you retain the Mayo Clinic services, and you can always restart your Rewards at any time by sending in your Wellness qualification form.

Q: What happens if I gain weight outside of my permitted range.
A: Even if you gain weight outside of your permitted range, you’ll save half the percentage you would have if you stayed within your weight range.

But since your life insurance policy may remain in force for many years, it is possible that the provider and/or menu of wellness services may change or be enhanced at some future date.
